An anal fistula is a painful and often recurring condition in which an abnormal passage forms between the inner lining of the anal canal and the skin around the anus. This condition can lead to discomfort, discharge, and repeated infections if not treated properly. Having a clear understanding of its causes and treatment options helps in managing it effectively.

An anal fistula typically appears as a small tunnel connecting the rectum or anal canal to the outer skin. It usually develops after an infection in the anal glands leads to an abscess. When the abscess does not heal completely, it may create a pathway that allows pus or fluid to drain continuously. Anal fistulas can be simple or complex, depending on their structure and depth.

Causes of Anal Fistula

The most common cause is an untreated or poorly managed anal abscess. However, several other conditions can also contribute to its development. Chronic inflammatory diseases such as Crohn's disease can increase the risk by causing persistent inflammation in the digestive tract. Conditions like Diverticular disease may also lead to infection and fistula formation. In some cases, infections such as Tuberculosis can damage surrounding tissues. Physical trauma due to surgery, injury, or childbirth may also play a role.

Treatment Options

The treatment of an anal fistula depends on how severe and complex it is. Initial management often focuses on controlling infection and ensuring proper drainage. A minor surgical procedure may be done to drain pus and relieve pressure. In some cases, a seton (a thin surgical thread) is placed to keep the tract open and allow continuous drainage while healing occurs.

For more complex or recurring fistulas, surgery is usually required. Procedures such as fistulotomy (removal of the fistula tract), fistula plug insertion, or advancement flap surgery may be performed to close the passage. If the condition is linked to underlying diseases like Crohn’s, medications may be prescribed to control inflammation and prevent recurrence.
